There is a new VCU/WIRB Approved Informed Consent Form (ICF)Template (dated 2/28/10).  The attached template can be found on the Office of Research Subject Project website at:  http://www.research.vcu.edu/forms/wirb.htm This template was revised at VCU’s request to include standard compensation for injury language which is intended to reduce review/negotiation by VCU and help ensure billing compliance.
